In December 2018 the Authority was advised by Police of concerns about a Police investigation that had been completed following a sudden death in the Canterbury Police District in 2015.
Police were concerned that the investigation did not comply with best practice and were undertaking a review of the investigation.
The Authority oversaw the Police review, which identified several investigative shortcomings and departures from best practice. Additional enquiries were also identified and completed prior to the conclusion of a coronial inquest.
